台灣老年人心血管疾病與痰瘀體質之相關性研究

【摘要】
背景:心血管疾病和痰瘀體質在老年人中很常見,很少有研究探討心血管疾病與痰瘀體質之間的關係。
目的:了解台灣老年人心血管疾病與痰瘀體質的發生比例,並探討影響老年人痰瘀體質的因素。
方法:本研究申請台灣人體生物資料庫資料進行次級資料分析,研究對象選擇60-70 歲間的2,353 位老年人。研究變項包括社會人口學資料、過去疾病史與痰瘀體質。痰瘀體質是透過中醫體質量表來評估。本研究採用多元邏輯回歸分析進一步確定影響痰瘀體質的因素。
結果:研究顯示台灣罹患心血管疾病的老年人為38.5%,痰瘀體質發生比例為8.5%,迴歸檢定統計顯示與痰瘀體質有顯著的因素包括女性、單身、肌肉骨骼疾病、肺部疾病、瓣膜性心臟病、冠心症或心律不整者。
結論:患有心血管疾病老年人有較高的風險存在痰瘀體質,尤其是女性。建議中醫師確診痰瘀體質時,關注老年人是否有其他西醫診斷疾病,並進一步安排疾病相關檢查,期望未來透過中西醫治療整合改善患者健康。

【Summary】
Background: Cardiovascular disease and stasis constitution are common in the elderly. There was rare study to explore the relationship between the cardiovascular disease and stasis constitution.
Objective: To understand the proportion of cardiovascular disease and stasis constitution in the elderly and explore the factors affecting the stasis constitution in the elderly.
Methods: This study is the secondary data analysis from the Taiwan Biobank Research Database. A total of 2,353 elderly with an age between 60 to 70 years old were selected. The study variables included the individuals’ demographics, past disease data, and body constitution, where the stasis constitution was assessed using the Body Constitution Questionnaire (BCQ). The factors acing most strongly for the stasis constitution was identified via multiple logistic regression analysis.
Results: The proportions of those elderly who had the cardiovascular disease and stasis constitution were 38.5% and 8.5%. The result of binary logistic regression showed female, single, people with musculoskeletal disease, lung disease, valvular heart disease, coronary heart disease or arrhythmia had a significantly associated
stasis constitution.
Conclusion: Elderly in Taiwan with cardiovascular disease had a higher risk for stasis constitution, especially for women. Chinese medicine doctors should pay attention to assess cardiovascular disease and arrange disease-related examinations when confirming the stasis constitution. We look forward to improving patient health through the integration of Chinese and Western medicine in the future.
