病例為一名23 歲女性罹患菊地氏病（Kikuchi-Fujimoto disease），主訴自2020 年9 月出現右側頸部腫痛、痛連肩部痠痛、伴隨發燒，服西藥Dexchlorpheniramine 2mg、Acetaminophen 500mg、Acyclovir 400mg 後已緩解，但雙小腿至腳背皮膚發紅紫不熱，自2020 年10 月發病起將近一年仍未消退，該患者於2021 年9 月至臺北榮民總醫院傳統醫學部中醫內科門診就診，中醫辨證為陰虛血熱、下焦溼熱，治則以滋陰降火、清熱利溼為主。中藥治療一個月後，雙小腿至腳背皮膚發紅紫逐漸減退，且同時改善便溏、白帶多⋯⋯等下焦溼熱的症狀。本病例報告顯示合併中藥治療菊地氏病，可有效緩解患者之不適症狀，可提供中醫同道中藥輔助治療之參考。

A 23-year-old female woman was diagnosed as Kikuchi-Fujimoto disease, and her main symptoms were that right neck was swollen and painful, shoulder pain, and fever since September 2020. These symptoms had been relieved after taking Dexchlorpheniramine 2mg, Acetaminophen 500mg, and Acyclovir 400mg.
The skin on both calves and insteps was red and purple without hot sensation, and the situation did not diminished during nearly one year since the onset in October 2020. Then she visited Center for Traditional Medicine of Taipei Veterans General Hospital on September 10, 2021. Due to the pattern of “Yin Deficiency and Blood Heat”, “Damp-Heat in Lowe-Jiao”, the Traditional Chinese Medicine (TCM) doctor adopted “Nourish Yin and Extinguish Fire” plus “Clean Heat and Dispels Dampness” to treat. After one-month treatment, the red and purple symptoms of both calves and insteps were effectively improved. At the same time, the symptom of “Damp-Heat in Lowe-Jiao” as loose stool, and white vaginal discharge were also improved. This case report showed that the integrated Chinese herbal medicine (CHM) and conventional medicine could effectively alleviate the complex symptoms of patients with Kikuchi-Fujimoto disease. Moreover, CHM
could be an alternative therapy for Kikuchi-Fujimoto disease.
